Returning Students
Welcome Back Returning Moc!
We are happy to hear that you are returning to FSC. If you are returning after leaving FSC for a semester or more, there are just a few more steps you need to take so we can pick up where we left off.
Re-Admission Forms
Standard Form
In order to resume taking classes with FSC you will need to fill out the following form.
Medical Break Form
If the reason for your departure was for health reasons, you will need to fill out the following form.
Academic Warning
In order to be readmitted to the College after withdrawing or attempting to return following a period away from the college with a grade point average of 2.0 or below, you will need to fill out the following form.
Transferring Credits
If during your time away from FSC you attended another institution, you must provide FSC with the official transcript(s) of these institutions.
